CBCHS is playing a key role in supporting people who test positive for COVID-19 to get the care they need through the COVID Positive Pathways program.
Working in collaboration with the Monash Health South East Public Health Unit, the CBCHS program has been operating for over 18 months and covers the entire south east catchment, operating seven days a week from 8.30am – 5pm.
The program is a Telehealth service managed by a team of nurses, allied health professionals, clinicians, GPs, support workers and administration staff who support patients with COVID-19 to access quality care and support at the right time and in the right setting.
Patients are triaged into one of four pathways (self-care, low-risk, medium-risk and high risk) based on severity of illness and social needs.
The Pathways team supports these patients to get the health care they require and the social and welfare supports needed to isolate safely. This may include providing care at home through a GP or health service or if required at hospital.
