Purpose of the Position

Primary Care Partnerships are a Victorian Government initiative aimed at improving the health of local communities through an integrated health promotion approach.

In the context of a social model of health, this position will support SMPCP initiatives to address Government and locally identified health priorities through the development of high quality priority setting, strategic planning, implementation and evaluation in accordance with the PCP vision, mission and strategic plan. This role will operate across five local government areas (Bayside, Glen Eira, Kingston, Port Phillip and Stonnington) with a population in excess of half a million  people.

Key Responsibilities

  • Provide support to SMPCP team members in applying a primary prevention approach to project work.
  • Apply place-based principles with key settings to implement health and wellbeing initiatives.
  • Work in collaboration with SMPCP partners, including but not limited to, councils, community health services and other providers to address identified population health issues.
  • Strengthen partnerships by utilising effective communication strategies to network and liaise with community agencies and key stakeholders regarding prevention focused SMPCP projects.

Key Selection Criteria

You are an energetic and self-motivated health promotion professional with experience working in the primary prevention space. You have skills in project management and community engagement and you are a highly effective communicator who can engage a wide range of stakeholders. Experience in primary prevention of violence against women, family violence or elder abuse will be considered an advantage.
