Diabetes

Service Description

The diabetes education team includes nurses, dietitians, podiatrists, and physiotherapists. We offer individuals with diabetes, their family friends or caregivers and opportunity to learn about diabetes and its management. Services available include comprehensive assessment, individual consultation, group programs and ongoing support to those who live within the City of Kingston

Diabetes education group programs are conducted on each site and run over 8 weeks, cost is $35

Topics covered include :

  • What is diabetes and pre diabetes
  • Medication
  • Blood glucose monitoring
  • Management of high and low blood glucose levels
  • The importance of exercise
  • Psychological impact
  • Foot care and prevention of injury
  • Dental health
  • Good health for the future
  • Good nutrition and dietary management

Podiatry, at all sites, offers “Diabetes & Feet” information sessions for $5 (free for partners & carers). It is compulsory to attend this group prior to Podiatry assessment & treatment.

Referrals

Preferable from health professional, self referral accepted.

Cost

Fees vary for individual clients, depending on their circumstances and the services provided.
