Community Health Nursing is directed toward individuals, families and groups. Community Health Nurses aim to assist people in the local community to maximise their health potential and well being through; Health Assessment, Counselling, Screening, Health Promotion/Education and Community Development. A community health nurse can assist people in accessing information, resources and services appropriate to their needs. Services that may be offered; pap tests, diabetes management, care for women during pregnancy, aged care, youth health, family planning and grief and loss

Support for Carers

Who are Carers: A Carer is a person (or persons) who are caring for someone in the community. The person being cared for may be of any age and suffer ill health, be frail aged or have a disability.

Care for carers can be in the form of personalized home visits from a Community Health Nurse to support and advise on caring issues, and to assist in referral to other services as required or the coordination of Carers Groups where carers can meet on a regular basis:

The aims of a Carers Group are

• to provide emotional support, information and practical advice to Carers,
• to improve the quality of life for Carers who are of risk of isolation and ill health due to their caring role
• to provide contact with other Carers,
• to provide a comfortable environment in which to discuss common issues relating to being a Carer
• to organise activities, guest speakers and themes of interest for Carers to enjoy.

Carers groups meet at Chelsea, Mentone and Clarinda. Each group meets monthly, Chelsea on the third Monday of the month, 1.30-3.30pm, Mentone on the first Friday on each month 10am-12md, and Clarinda on the third Wednesday of the month 11am-12.30pm. Phone for further information if needed.
